Webinar: STEADI Implementation and Partnering with Health Care

Learn about how the Centers for Disease Control and Prevention’s STEADI (Stopping Elderly Accidents, Deaths and Injuries) Toolkit can be used for fall prevention screening, assessment, intervention and education of older adults. New developments related to STEADI, such as integration with electronic health records, will be discussed. Examples of how the STEADI Toolkit is being embedded into clinical practice and emergency medical services will be described.

Presentation: Evaluation of Oregon’s Community Health Coalition

Oregon developed and evaluated a community health coalition, entitled “Sustainable Relationships for Community Health.” This evaluation describes the coalition and it’s effectiveness in serving as a learning cooperative for members, improving cross-sector partnerships, and developing tools to enhance community health partnerships.
