This sample Medicare preventive services calendar was developed by the Virginia Insurance Counseling & Assistance Program (VICAP). The calendar highlights different free preventive services available to people with Medicare. In addition, it contains a tear-off sheet at the back of the calendar that asks for contact information and monthly income with an addressed envelope so that beneficiaries can send it to the VICAP to begin the screening process for Extra Help.
