Both Medical Alert and Medical Guardian sell medical alert systems in home units, wearable GPS necklaces, and smartwatches. Medical Guardian goes one step further to offer a GPS wristband that’s not a watch. The only wristband Medical Alert offers is the one that comes with the home unit.
Speaking of home units, the Medical Alert Home System has a range of 800 feet, which is on the shorter side of average for medical alert systems. Medical Guardian’s home unit range is 1,400 feet (about ¼ mile).
Medical Guardian charges for its added tech and longer range, costing a few dollars more per month than Medical Alert. Where the two brands align is fall detection: It costs $10 to add fall detection to your devices, no matter which brand you choose. Both brands include the option for a lockbox to keep a set of keys for emergency responders to use. With Medical Alert, the lockbox is free with an annual plan.

Our testing experience with Medical Guardian
Medical Guardian took the top prize out of all the medical alert systems we tested. It cemented this title because it had great care partner features and customer service. Medical Guardian even pulled an impressive 9.4 out of 10 for user experience alone. Every time we called the Medical Guardian monitoring center, the staff was patient and understanding that we were conducting a test.
Device setup was simple. And the help buttons were easy to test—as were the batteries. Medical Guardian scored 5 out of 5 across the board, except for wearability. Our testers found the MG Mini Lite (the non-watch wristband) to be a bit obvious. “It’s clearly a medical alert device,” a tester said.

Medical Guardian’s customer service and warranty
Medical Guardian doesn’t offer a free trial period, but it does offer a warranty. The one-year manufacturer’s warranty covers a device replacement if your device is defective. The warranty does not cover spills, drops, or normal wear-and-tear damage.
You can cancel your Medical Guardian service any time without a fee. The company will prorate your refund for the months you didn’t use. You’ll need to return any equipment you didn't purchase, though. And there may be a restocking fee.

Our testing experience with Medical Alert
A medical alert system that's simple to use—like Medical Alert proved to be—can bring quick help in case of a fall. This is especially important considering 20-30% of older adults who fall end up with "moderate to severe" injuries such as bruises, hip fractures, or head trauma.2
Our testers had no issues unboxing Medical Alert's system and scored it a 5 out of 5 for setup. “There isn’t a lot of extras. I found it easy to understand what everything is,” said one of our testers, going on to compliment the dexterity-friendly packaging. The lack of call cancellation is the only reason the Medical Alert Mobile system got 4 stars instead of 5. The help button is large, and easy to hit on accident. Call cancellation would have been a nice feature to have, given that 20% of our medical alert system survey respondents reported feeling nervous about setting their alarms off accidentally.1
In the wearability category, the Mobile system earned 3 out of 5 stars for being fairly noticeable. That said, our testers found some positives. One noted the system is “comfortable, light, and doesn’t bump or bounce a lot when I’m walking."
The wristband on the Medical Alert At Home system presented a few accessibility concerns. “When I went to adjust it, it slipped out of the clasp altogether," said one of our testers. "I needed to re-thread it, which would be difficult [for someone] with dexterity issues.”

Medical Alert does not offer a trial period. You can add a protection plan for $1 per month. The company’s warranty covers manufacturer's defects.
