Our process testing the best mattresses for hip pain 

Our team has spent thousands of hours researching and testing mattresses so you can feel confident in our reviews. We work with experts in sleep, physical therapy, and occupational therapy, so we know what features are most important when searching for a mattress to ease hip pain. 

For this review, we personally tested 30 mattresses identified through our research to offer pain relief. Each mattress was put through a rigorous testing process by older adults with pain, to ensure we have real-world data about how they perform. To find the best mattresses for hip pain, we tested mattresses using pressure-sensing devices and other equipment such as heat measurement guns. We also recruited older adults to test the mattresses and report on their experiences, ranking firmness, edge support, pressure relief, and other factors. 

Optimal firmness levels

For people with chronic, nonspecific low back pain, research has found that medium-firm mattresses are best, and this likely affects hip pain in a similar way.9  Sleeping on a medium-firm mattress (6.5-7) was found to improve sleep quality by 55% and decrease back pain by 48%. People with a higher weight tend to perceive mattresses as less firm than they are, so may want to choose a firmer model. 

Pressure relief technologies

Our pressure relief score reflects our testers’ experiences on the mattress and the results shown on our pressure map. The pressure map uses sensors to show how much pressure the body feels when lying on the mattress. Blue and green are great pressure relief; yellow, orange, and red show places where pressure builds up. We looked for mattresses showing low pressure around the hip area.

Zoned support 

Some mattresses are made with extra support in key areas, such as the hips. This helps reduce pressure on those areas and keeps the spine straight. We looked for zoned support that cushions the hip joint.

Spinal alignment

People with hip pain need extra support around their hips and lower back. We looked for mattresses that are supportive in those areas and keep the spine straight while offering enough cushioning to relieve pressure on the joints. We asked testers how they felt lying on their side and had them lie on each mattress to understand the level of support the mattress provided to people of varying weights.

Cooling 

“Some types of hip pain may be caused by inflammation or swelling in the circulatory system,” explained Heather Swain, a doctor of physical therapy who specializes in pain and owns Ally Total Physical Therapy in Toledo, Ohio. In those cases, a cooler mattress may help. We used a heat gun to measure the temperature of each mattress before and after lying on it, and had testers note how it felt to them. 

Sleep positions and hip pain

“Sleep position is very difficult for people to change," said Swain. “You’re fighting an unconscious person’s preferences.” It also matters what part of the hip the pain is focused on: the side, or the back near the glutes. But there are some ways you can work with whatever position you are most comfortable in. Here are some tips for sleeping with hip pain in any position:

  • Side Sleepers: “If your hip pain is on the side of your hip, you can try switching to the opposite side. If that doesn’t work, look for a mattress with zoning in that area to take some of the pressure off the joint,” Swain said. She has also used pillows between the knees to help widen the angle of the hips and release some of the tension there. 
  • Back Sleepers: If you have hip pain from sleeping on your back, you want extra support there. Placing a pillow under your knees can help to round your back out and keep your spine straight. 
  • Stomach Sleepers: Sleeping on your stomach puts your back in an extended position. You can even this out with a thin pillow or no pillow under your head, or by adding a pillow or a folded towel under your hips.

6 ways to sleep better with hip pain

One of the best ways to address your hip pain is to understand its cause. “It could be arthritis, sciatica, a back problem that’s radiating to the hip and causing pain,” said Swain. “Ultimately, figuring out the cause is the thing that will be the most useful to fix it.”

Discovering the cause of your hip pain requires consulting appropriate health care professionals, because the hip is a complicated joint. “A lot of the muscles that rotate and move the hip attach at that point,” said Swain. 

Whether you’re dealing with arthritis, bursitis, injury, or other causes of hip pain, trying to achieve comfortable sleep can be a challenge. To reduce sleep disruptions from hip pain and improve your sleep quality, try the following tips: 

  1. Gentle stretching before bed: Lightly stretching tight muscles, including the quadriceps, hip flexors, or hamstrings, can provide you with tension relief and improve your flexibility and joint health.10 
  2. Apply hot or cold therapy: Applying a heating pad or warm compresses to your painful hips can reduce stiffness, while an ice pack wrapped in a towel can ease inflammation. 
  3. Pillow positioning: Side sleepers may benefit from placing a pillow between the knees and ankles, which can reduce strain on the hips. Back sleepers may try sleeping with a pillow under their knees, relieving tension in the lower back and front of the hips. Try adjusting your pillows until you find the best setup for your pain. 
  4. Try a weighted blanket: If not too heavy, a weighted blanket can reduce tossing and turning, adding more stress to painful joints.
  5. Aim for a neutral spine: “Sometimes,” Swain explained, “Rolling up a towel in the crook of your hip will promote more neutral spine alignment and support the hip better due to even pressure distribution. There’s not as much weight pressing down on the bony part on the side of your hip.”
  6. Practice good sleep hygiene: Lack of sleep can make your joint pain feel worse. Consider a wind-down routine to prepare your body and mind for bed. Keep a regular sleep schedule. And use other sleep hygiene tips to help you fall asleep faster.

The average mattress lifespan is around 7 years, according to the Better Sleep Council.12 How often you need to replace a mattress can vary depending on the mattress construction, use, and any mattress protector or pillow topper.

Can a mattress really help with hip pain?

A mattress is no substitute for proper medical care, but it may help with some kinds of pain, depending on the cause. In some cases, added support in the hip region may help relieve pressure on the joint. A cooling mattress may help with inflammatory causes of hip pain. While it should not be your only action, changing your mattress can be part of a comprehensive treatment plan to provide more relief and comfort for hip pain.
