Most older adults are living with chronic diseases. An estimated 93% have at least one, and nearly 79% live with two or more.

NCOA receives funding from the Administration for Community Living/Administraiton on Aging (ACL/AoA), which awards grants to state and tribal organizations to expand and sustain chronic disease self-management education (CDSME) programs arcoss the country. These evidence-based programs are proven to help older adults manage chronic conditions and improve their quality of life.
