Evidence-Based Programs: Health in Motion - Digital Fall Risk Reduction Program

The program includes digital versions of the A Matter of Balance and Otago Exercise Program, along with standardized fall risk assessments like the CDC STEADI tools and functional tests. It features self-guided, avatar-led exercises and is delivered virtually by health coaches through a smart device, tablet, or computer. All data feeds into individual and population dashboards, allowing providers, senior centers, and AAAs to expand their reach and monitor participant progress remotely. The program is fully virtual and customizable to individual needs.

Target Audience: The program targets any adult aged 55 and older who is at risk of falling or experiencing a decline in balance/function and would benefit from falls efficacy, fall risk reduction, and/or fall prevention programs. It also supports falls prevention administrators seeking to expand the reach of their current offerings and streamline data collection for existing programs.

Health Outcomes:

  • 71% of participants completed the full digital A Matter of Balance program.
  • Participants showed lasting improvements in:
    • Balance confidence
    • Lower body strength and mobility
    • Reduced fear-related avoidance behaviors
  • Nearly 500,000 exercise repetitions were completed over 12 months.

Delivered By: 1 Trained Lay Leader/Facilitator, Certified Health Coach, Physical Therapist, Occupational Therapist, Trained Care Consultant, Nurse, Certified Fitness Instructor, Social Worker/Mental Health Therapist, Community Health Worker; A single program administrator could monitor the program for hundreds of participants independently.

Training: Leader/coach training is available remote/virtual, in-person, or on-demand and typically takes about 10 hours, which can be spread over several days and/or weeks.

Program Type: Individual; this program is entirely self-guided with oversight by the program administrator.

Format: Remote (Virtual Programming/In home)

Length: The AMOB portion of the program can be completed in less than 3 months. Otago is a program designed to be completed over the course of 12 months; thus, we recommend that participants continue exercising for the 12-month duration.

Data Collection Requirements: Data is captured via the app and stored on our servers. We use the de-identified data to improve the program. MaineHealth requests that de-identified data outcomes be shared quarterly, in a report format.
