Finding the right mattress can have a significant impact on your sleep quality. This is especially true for older adults dealing with joint pain and arthritis. Studies show that more than 53 million adults [1] Centers for Disease Control and Prevention. National Statistics—Arthritis Prevalence. Oct. 24, 2023. Even cheap mattresses can cost a significant amount of money for some people, and the mattresses we tested are not cheap—in neither construction nor cost. Our top pick

Our expert take on the Saatva RX mattress

This 15-inch-thick medium-firm mattress combines luxurious comfort with exceptional support, making it an excellent choice for anyone seeking relief from joint pain or discomfort.

The Saatva RX uses a unique coil system with reinforced coils in the center. This design helps maintain proper spinal alignment throughout the night, which is crucial for reducing pain and promoting better sleep. The added layer of high-density memory foam specifically targets the lower back, enhancing overall support. It’s no surprise that our Saatva testers gave this mattress a perfect score of five out of five for overall support in all sleeping positions.

Another standout feature is the edge support, where it also scored a perfect five out of five. Our tester noted that it was easy to get in and out of bed, making it suitable for people experiencing limited mobility due to chronic pain or who are at risk of falling

Strong edge support also allows for safely sleeping very close to the edge, as some sleepers prefer to do. Our tester said they felt secure lying near the edge. “You could be blindfolded and not realize you were lying at the edge,” they said, adding that they “literally” hung over the edge of the bed and did not fall or slide off. Edge support this strong is a highly valuable feature for older adults who are concerned with preventing a fall.

This mattress also has high scores (4.75 out of five) for pressure point relief, the most important metric for people with joint pain. This is due to the combination of foam caps on each spring and an additional micro-coil layer that ensures the mattress adapts to your body’s contours, regardless of your preferred sleep position. Back sleepers, in particular, will appreciate the perfect balance of firmness and cushioning, while side sleepers will find adequate pressure relief for the shoulders and hips.

Cooling and motion isolation The ability of a mattress to keep movement on one side of the bed from affecting the other side are also top-notch (4.5 out of five), thanks to the breathable materials and coil construction of the Saatva RX, which allow for ample airflow. Our tester said they could barely feel their sleep partner change positions.

Its high 4.4 out of five functional support NCOA’s rating of suitability for older adults who may have limited mobility. Calculated using firmness level (30%), overall support score (30%), edge support score (30%), and bounce score (10%). score speaks to its suitability for older adults who will appreciate the exceptional overall support, bounce, and edge firmness.

One of the highlights of purchasing a Saatva RX is the free white-glove delivery service. This means you won’t have to worry about the hassle of setting up your new mattress or disposing of your old mattress, a significant advantage for those living alone or dealing with physical limitations. Also, the Saatva RX uses eco-friendly materials like organic cotton in its cover and recycled steel for its coils. These sustainable choices help provide a healthier sleep environment and reduce environmental impact.

Due to the highly specialized nature of the construction and materials in this mattress, it is the most expensive in our round-up, costing $2,945 for a queen size. But, if it’s in your budget, we are confident it is an excellent choice for people living with chronic pain.

Customer reviews

The Saatva RX has 4.9 out of five stars on Saatva’s website with 73 customer reviews.

“Our new Saatva Rx mattress is either magic or a miracle! I haven’t slept comfortably for a very long time due to chronic, severe hip issues and low back pain. Recently, a couple of friends/family members told us about Saatva and their excellent experiences. We took the “mattress quiz” on the website and decided to purchase the RX. I was almost afraid to be hopeful, even though I’m only in my 50s, that I would ever sleep through the night without pain and discomfort again…but the very first night was a godsend! I almost cried when I opened my eyes in the morning—not from being in pain but because for the first time in years I didn’t wake up once during the night hurting! Lastly, we’d like to praise the delivery and set-up team. They were friendly and communicative about their arrival time and had our old mattress (and topper) out and the new one in and set up quickly.”

 — KM., verified reviewer, on 6/7/2024

“Fantastic mattress for people with back issues. I have had spinal surgery and my husband suffers from lower backs issues. We have always spent $$$ on our mattresses. This has comfort with just the right amount of firmness. We both slept well from the first night..”

Our expert take on the Helix Dusk Luxe mattress

Our testing of the Helix Dusk Luxe revealed why it stands out as the most comfortable orthopedic mattress. This plush yet supportive mattress strikes an ideal balance, making it perfect for those who need comfort without sacrificing support. It scored a firmness rating of seven out of 10, leaning slightly toward the firm side while still offering a soft, luxurious feel.

The Helix Dusk Luxe features a quilted cover and two layers of cradling foam that provide excellent pressure relief (4.5 out of five), an important feature for people living with pain. Our testers found it to be comfortable in all sleep positions, particularly for side sleepers who need extra contouring around the hips and shoulders. Our testers also found the mattress’s responsiveness impressive, thanks to its zoned coil support. It scored a 4.5 out of five for bounce. Its high bounce translates to lower motion isolation The ability of a mattress to keep movement on one side of the bed from affecting the other side. 1, where it scored a three out of five average rating. It may not be the best for sleepers who are easily disturbed by a partner’s movements.

Edge support was less robust, scoring a 3.5 out of five, but still adequate. The mattress’s cooling capabilities also scored high, making it a great option for those who tend to sleep hot. Its overall functional support, taking into account firmness, overall support, bounce, and edge support, rates at a 3.75 out of five. This falls between average and good for functionality for older adults.

WinkBeds offers $50 off for current and former U.S. military and their families, and first responders, including law enforcement, fire, and health care workers.

Our expert take on the WinkBed mattress

The WinkBed is our choice as the best orthopedic mattress for couples. We tested the luxury firm version, which scored a 6.5 on the firmness scale, putting it at universal medium. It also comes in three other firmness options: softer, firmer, and plus.

This hybrid mattress features a moisture-wicking eucalyptus cooling cover; a Euro pillow-top layer of contouring, gel-infused foam; and a layer of zoned, individually wrapped coils; all of which led to high performance scores from our testers.

Most notably, our testers found the WinkBed’s motion isolation impressive (four out of five), making it an excellent choice for couples. The individually wrapped coils limit motion transfer, ensuring movement on one side of the bed will not disturb the sleeper on the other side. Our water-glass test showed barely any movement when someone sat on the other side of the bed.

The same coil system led to high support and pressure relief scores as well (four out of five and 4.5 out of five, respectively). The zoned coil support system adapts to different sleeping positions, maintaining spinal alignment and providing excellent pressure relief, especially for back and side sleepers.

The mattress also excels in cooling (four out of five), thanks to its breathable materials and construction that promote airflow. Edge support is robust, giving a stable and secure feel when sitting or lying near the edge.

Bounce and responsiveness is always a trade off with motion isolation, so it’s no surprise this is the WinkBed’s lowest score—although it is still above average at a 3.5 out of five. Although our tester did not have a difficult time changing positions on this bed, they noted that the pillow top may compress slightly, making it a tad bit harder to move for some.

The 3.7 out of five functional support score makes this mattress suitable for most older adults who may have more limited mobility, thanks to its high overall and edge support.

The pricing is on the lower end for orthopedic mattresses, so this is a great option for most older adults with chronic pain.

Our expert take on the Bear Elite Hybrid mattress

The Bear Elite Hybrid is our choice for the best orthopedic mattress for pressure relief. This mattress provides excellent comfort for those dealing with hip pain or other pressure point sensitivity, thanks to its high pressure relief rating and gentle zoned support.

Our testers gave the Bear Elite Hybrid a perfect five out of five score for pressure relief, which will be especially appreciated by side and back sleepers. The plush cover, combined with two layers of memory foam, ensures the mattress contours well to sharper pressure points like the shoulders, providing a cradling and comfortable sleep experience. This mattress’s gentle zoned support helps maintain spinal alignment, which is crucial for preventing irritation and discomfort in sensitive areas like the hips and shoulders.

This mattress also performs exceptionally well in cooling (4.5 out of five score), thanks to phase change materials (PCMs) A material that stores or releases heat to maintain a constant temperature. , Celliant Celliant contains naturally occurring minerals and elements woven into fiber, making a product able to capture a body’s heat and convert it into infrared energy that goes back into the body. fiber, and copper-infused memory foam.

Edge support is adequate, providing stability when sitting or lying near the edge. The mattress’s motion transfer capabilities are also above average, making it a good choice for couples who want to minimize disturbances from their partner’s movements. Our Bear tester noted that the cover actually felt cool to the touch.

While the mattress offers great pressure relief and comfort, its overall support and edge support are more moderate, scoring 3.5 out of five and three out of five, respectively. This means it may not provide the ideal level of mobility for higher-weight sleepers, especially those who require a firmer mattress for stomach sleeping. Also, the lower edge support might make it harder to get in and out of bed for some. These factors led to a slightly higher than average functional support score of 3.23 out of five. While it should suffice for lighter-weight sleepers, higher-weight sleepers may have trouble changing positions and getting in and out of this bed.

Our expert take on the Nolah Evolution mattress

The Nolah Evolution is our pick for the best orthopedic mattress for back pain. This 15-inch high mattress provides a balanced mix of support and comfort, making it the best mattress for lower back pain. We tested the luxury firm model, which gives a gentle firm feel, scoring a 7 out of 10 on the firmness scale. It also comes in plush or firm models.

Our testers were impressed by the double-zoned support system, featuring Nolah’s exclusive AirFoam, which is perforated Perforated foam or latex is designed by drilling holes through the material to increase the breathability and cooling capabilities of a mattress, as well as to increase the softness of the foam. to provide tailored levels of compression and support for all areas of the body. The foam compresses more at the shoulders and hips for pressure relief and remains denser at the lower back for extra support. This design, along with a layer of zoned coils reinforced in the center, ensures the mattress will support the lower back and help maintain spinal alignment. It received an “excellent” 4.75 out of five for pressure relief, and a “good” four out of five for overall support. It also excels in edge support (4.5 out of five score), providing a stable and secure feel when sitting or lying near the edge.

Bounce and motion isolation are equal, both scoring 3.5 out of five, something we rarely see in testing. Usually a mattress is high in one or the other, but the Nolah Evolution was above average in both. Our Reviews Team believes it would make a decent choice for couples. While larger movements like getting into bed may be noticeable, smaller movements are subtle, ensuring relatively undisturbed sleep for all but the lightest sleepers. Lighter-weight sleepers should have no problem moving around on it, while higher-weight people may feel somewhat stuck in the material.

This mattress has a relatively high 3.95 out of five functional support score due to its firmer feel, high edge support, higher-than-average bounce, and for its overall support.

Cooling performance is only average, so we would not recommend this mattress to people who sleep hot. It’s priced on the lower end for orthopedic mattresses, making it an excellent value.

Our expert take on the Brooklyn Bedding Aurora Luxe mattress

The Brooklyn Bedding Aurora Luxe is our choice for the best orthopedic mattress for side sleepers due to its exceptional pressure-relieving capabilities, which help avoid pain at the hips and shoulders when lying on your side—a common complaint for side sleepers.

This mattress offers a firmer feel, while providing excellent pressure relief, making it ideal for those who need a supportive but soft sleep surface. While it comes in three firmness levels, we tested the medium and gave it a rating of 7.5 out of 10 on the firmness scale. This ranked it solidly at a “firm” level, even though the company describes it as “medium.”

It’s rare for a firmer mattress to exhibit superior pressure relief, but the Aurora Luxe It manages to balance firmness and comfort, making it suitable for all sleeping positions.

The mattress gets its pressure-relieving capabilities from a combination of conforming and responsive foam layers, which help distribute weight evenly and ease pressure on the shoulders and hips. Its zoned support system ensures that the spine remains in a neutral position, reducing pain and discomfort.

Edge support is also strong (4.5 out of five), providing a secure and stable feel when sitting or lying near the edge. The cooling performance of the Aurora Luxe is good, with a four out of five score. The first foam layer is infused with PCM—as we saw in the Bear Elite Hybrid—which also had a high cooling score. This makes the mattress feel cool to the touch and keeps even hot sleepers comfortable throughout the night.

This Aurora Luxe has average bounce, so should suit lighter-weight and average-weight sleepers, but higher-weight sleepers may have more trouble changing positions on it. The mattress’s lower-than-average motion transfer score indicates that it may not be the best choice for couples who are sensitive to each other’s movements during the night.

How we test orthopedic mattresses

Our Reviews Team has dedicated more than 1,000 hours to researching the mattress industry, consulting at least 10 health care professionals, and testing roughly 40 brands and 130 unique mattress models for quality and comfort. We’ve also interviewed eight older adults who recently purchased a mattress and surveyed more than 3,200 recent mattress customers to understand their experiences shopping for a new bed.

Each mattress undergoes a rigorous 36-page testing procedure to score its different characteristics, including firmness, support, and other aspects most shoppers consider. We choose the best orthopedic mattresses based on the scores of the relevant criteria. See how we tested the most important considerations for this review below.

Firmness

Like most mattress brands in the industry, we use a one-to-10 scoring method when testing firmness, with 6.5 being the true medium firm. Our testers rate this by lying on the surface and comparing the feel to other mattresses we’ve tested. Material density and coil strength influence firmness the most. For orthopedic mattresses, a firmness score of 6–7 is often ideal, providing enough support without being too rigid. A score of 6.5 is considered a universal medium and is most comfortable for a wide range of people.

Pressure Relief

We rate pressure relief on a scale of one to five, with five being the most pressure-relieving. We consider how it feels and use a pressure map with sensors to visualize pressure points around the body accurately. Blue and green pressure map results show great pressure relief, while yellows, oranges, and reds indicate pressure build-up. Orthopedic mattresses need to provide enough sinkage and pressure relief to ensure comfort and reduce pain in sensitive areas.

Support

We score support on a scale of one to five, with five being the most supportive. Mattresses with strong support contour and push into the curves of your body, particularly the lower back or midsection. Not only do we feel support during testing, but we can also visualize it through pressure map results. Maps with mostly white space at the lower back show how the mattress isn’t making contact with the area, which means the mattress isn’t supportive. Orthopedic mattresses generally provide extra support around the lumbar (lower back) region, so mattresses with zoned support are preferred.

How to choose an orthopedic mattress

Choosing the right orthopedic mattress is crucial for those with chronic pain or orthopedic issues. The right mattress can significantly enhance sleep quality, reduce discomfort, and promote overall well-being. Here are the key factors to consider when selecting an orthopedic mattress:

Firmness

Firmness is a critical factor in choosing an orthopedic mattress, as it directly affects the level of support and comfort you will receive. For people with chronic pain, a medium to medium-firm mattress is generally recommended. These mattresses provide a balance of comfort and support, ensuring that the body is properly aligned without feeling too rigid. A mattress that is too soft can cause the body to sink excessively, leading to misalignment and increased pain, while a mattress that is too firm may not provide adequate contouring to relieve pressure points.

When considering firmness, also think about your body weight and sleeping position. Heavier individuals might require a slightly firmer mattress to prevent excessive sinkage, while lighter individuals may prefer a softer feel to ensure proper contouring. Side sleepers often benefit from a mattress with a bit more cushioning to ease pressure on the shoulders and hips, whereas back and stomach sleepers typically need firmer support to maintain spinal alignment.

Support

Support is another crucial aspect when selecting an orthopedic mattress. Proper support ensures that the spine remains in a neutral position, which is essential for reducing pain and preventing further injury. Orthopedic mattresses often feature zoned support systems, which provide varying levels of support across different areas of the body. These systems are designed to be firmer in the middle section of the mattress to support the lower back and hips, and softer at the head and feet to provide comfort and relieve pressure points.

Edge support is also important for people with chronic pain, especially those who may have difficulty getting in and out of bed. A mattress with strong edge support prevents sagging and provides a stable surface when sitting or lying near the edges. This can enhance overall safety and ease of movement, reducing the risk of falls and making it easier to get up from the bed.

Pressure relief

Effective pressure relief is essential in an orthopedic mattress, particularly for individuals with chronic pain. Pressure relief refers to the mattress’s ability to distribute body weight evenly, reducing stress on sensitive areas like the shoulders, hips, and lower back. Memory foam and latex are popular materials for providing excellent pressure relief, as they conform to the body’s shape and offer targeted support.

When evaluating pressure relief, consider how the mattress responds to your body’s contours. A good orthopedic mattress should cradle your body while maintaining enough firmness to support the spine. Look for a mattress that provides consistent support and relief across all sleeping positions to ensure a comfortable and pain-free sleep experience.

Frequently asked questions

Doctors typically recommend a medium-firm to firm mattress for back pain, as it provides the necessary support to maintain proper spinal alignment and reduce pressure point discomfort. Orthopedic mattresses with zoned support systems and memory foam or latex layers are often suggested, as they offer targeted support and pressure relief to reduce back pain.

A spring mattress uses a system of coils to provide support and a bouncy feel, but it may lack the targeted pressure relief needed for certain pain conditions. An orthopedic mattress, on the other hand, is specifically designed to support spinal alignment and relieve pressure points. While its construction usually contains a spring coil system, it often features advanced materials like memory foam, latex, and zoned support systems to offer enhanced comfort and support for those with orthopedic issues.

The best brand of orthopedic mattress can vary based on individual needs and preferences, but some of the brands that have scored highest in our testing include Saatva, Helix, WinkBed, Bear, Nolah, and Brooklyn Bedding. Each of these brands offers orthopedic mattresses designed to provide targeted support, pressure relief, and enhanced sleep quality for various types of sleepers.
