Having a healthy sex life can improve your mental health and your overall quality of life. [1] National Institute of Diabetes and Digestive and Kidney Diseases. Definition & facts for erectile dysfunction. July 2017. Erectile dysfunction (ED) can disrupt your mental, physical, and sexual health. In our January 2024 survey of men* with ED, over 76% of respondents reported that ED impacted their mental health negatively. That’s why treating erectile dysfunction is so important. Pros & cons
Pros Approval or denial within 24 hours Discreet and fast shipping Turns away users who are ineligible for treatment Free consultation and follow-ups Cons Medication pricing is confusing Doesn’t offer brand-name Cialis More expensive than other online platforms
Our expert opinion

Overall, Rex MD is a reputable platform with some decent qualities, but its pricing structure is confusing, and its medications are more expensive than other platforms.

We were impressed by how the intake questionnaire stops you if there’s any reason you shouldn’t take erectile dysfunction medication. For instance, you shouldn’t take erectile dysfunction medication if you are taking nitrate medications or if you have used the recreational drug known as “poppers” within the last three months. We appreciated that Rex MD flagged these concerns since it’s unlikely that most users are aware of who shouldn’t take ED medication.

Rex MD also doesn’t recommend you continue with ED treatment if you haven’t seen your primary care doctor within the last three years. “A lot of these online platforms cater to people who don’t necessarily have a problem but just want to perform better. It’s an opportunity for providers to intervene early and prevent issues,” said Amy Pearlman, MD, urologist and owner of Prime Institute in Miami, Florida. Receiving comprehensive care through your health care provider can help you find and treat other serious conditions that may lead to ED, such as prediabetes, high cholesterol, testosterone deficiency, and decreased fertility.

The intake questionnaire stops you if there’s any reason you shouldn’t take erectile dysfunction medication or are not recommended for Rex MD

While we liked the safety-conscious sign-up process, we found the cost of ED medications difficult to find and confusing to break down. On the home page, Rex MD lists the starting costs of each medication without going into further detail about dosing. To learn more about the various medication costs, you must complete the sign-up process, which includes entering your email address and phone number to create an account.

We also discovered that Rex MD is not the most affordable online platform for ED. Its website guarantees the lowest price per tablet compared to other online platforms, but this claim isn’t true. Hims offers a lower cost for some of the same medications at similar doses. For instance, Hims charges $1.06–$2.17 per pill for as-needed generic Cialis in a 20 mg dose, while Rex MD charges $6–$10 per pill.

Special considerations of erectile dysfunction medication for older adults

Viagra and Cialis may be ineffective or dangerous based on your health history and current medications.

Some of the most common chronic conditions affecting older adults—including high blood pressure, high cholesterol, heart disease, and diabetes—can damage penile blood vessels and nerves, leading to new or worsening ED that doesn’t respond to Viagra or Cialis [4] Mayo Clinic. Erectile Dysfunction. . Fortunately, several other ED treatments are available, including penile injections, vacuum erection devices, and a discreet penile prosthesis. [5] McMahon CN, et al. Treating Erectile Dysfunction When PDE5 Inhibitors Fail. BMJ. March 11, 2006.

ED can also appear as one of the first symptoms of cardiovascular disease, hypertension, anxiety, and other chronic health concerns. [6] Capogrosso P, et al. Erectile Dysfunction in Young Patients is a Proxy of Overall Men’s Health Status. Current Opinion in Urology. 2016. If you haven’t had a physical exam within the last year, don’t try to get ED medication online. Visit your primary doctor in person to ensure underlying conditions are diagnosed and treated alongside ED.

Who shouldn’t take Viagra or Cialis

Viagra and Cialis shouldn’t be taken with some of the medications used to treat chest pain, high blood pressure, or lower urinary tract symptoms. Viagra should also be avoided in men taking protease inhibitors—a specific class of medications used to treat HIV.

Combining nitrates or alpha-blockers with ED pills may cause a severe, life-threatening drop in blood pressure, which can be associated with feeling dizzy, fainting, falling, and even death.

This is not a complete list of medications that should not be taken with Viagra and Cialis. Talk to your provider about everything you take—including recreational drugs—when discussing potential ED treatments.

Cialis and Viagra also shouldn’t be taken by people with low resting blood pressure, uncontrolled high blood pressure, inherited disorders of the eye or retina, recent heart attack or stroke, or uncontrolled heart disease. Cialis and Viagra may exacerbate abdominal pain in men with stomach ulcers.

What is Rex MD?

Rex MD is a telemedicine platform catered toward men’s health that prescribes medications to users without the need to see a doctor in person. A health care provider will review your medical information and medication request and approve or deny it. The medication will be shipped to your home in discreet packaging within two to four business days if approved. You won’t meet with your Rex MD provider for a live consultation, but you can message them about your prescription.

In addition to ED, Rex MD provides treatment for:

Rex MD complies with the Health Insurance Portability and Accountability Act (HIPAA) and is certified by LegitScript, a third-party company that reviews and monitors health care platforms to ensure they comply with applicable laws and regulations set by organizations such as the U.S. Food and Drug Administration (FDA).

Rex MD offerings

Rex MD carries generic Cialis (tadalafil), generic Viagra (sildenafil), and Viagra. Unlike other online ED platforms we’ve reviewed, they do not offer brand-name Cialis. Many people prefer the generic because it’s more affordable, but if you want brand-name Cialis, you’ll have to look elsewhere. They also do not carry newer ED medications like Stendra (avanafil) or Levitra (vardenafil).

Rex MD has free and discreet shipping. If you ever have any questions or concerns about your medication, you can message the provider who viewed your intake survey and approved your prescription.

Rex MD offers a “two-night free sample” of generic Viagra (50 mg), which can benefit people who want to try ED medication before buying a large quantity. However, the offering is not completely free, costing $4.95 for shipping. Keep in mind that while most men respond to lower doses of ED medication, some require increasing doses before the medication is effective. [7] Zhonglin C, et al. Practical Approaches to Treat ED in PDE5i Nonresponders. Aging and Disease. October 2020. So the two-pill sample may or may not be effective for everyone.

Cialis (tadalafil)

Rex MD offers generic Cialis (tadalafil) in 20 mg and 10 mg doses. If you plan on taking your medication as needed, you can take tadalafil one to two hours before sex and it will last for 18 hours. [8] Zucchi A, et al. The First-Generation Phosphodiesterase 5 Inhibitors and Their Pharmacokinetic Issue. Andrology. July 2019.

Some people choose to take tadalafil daily at a low dosage rather than as needed because it allows for more spontaneity in sexual activities. With a daily dose, you don’t need to schedule to take the medication before sex. Rex MD offers daily tadalafil at 2.5 mg and 5 mg. [8] Zucchi A, et al. The First-Generation Phosphodiesterase 5 Inhibitors and Their Pharmacokinetic Issue. Andrology. July 2019. [9] Washington SL, Shindel AW. A Once-Daily Dose of Tadalafil for Erectile Dysfunction: Compliance and Efficacy. Drug, Design, Development and Therapy. 2010.

Research shows that some men who have severe erectile dysfunction symptoms may benefit more from as-needed doses compared to daily dosing. If you have severe symptoms, you should see your health care provider.

Viagra (sildenafil)

Generic Viagra, also called sildenafil, starts working 30–60 minutes after being taken. [10] MedlinePlus. Sildenafil. The effects generally last up to four hours. Sildenafil should be taken on an empty stomach. If you do eat before taking sildenafil, avoid foods that are high in fat because it decreases the medication’s effectiveness. Through Rex MD, Viagra is available in 50 mg and 100 mg dosages.

Rex MD offers a cost-effective sample pack of generic Viagra. On its website, Rex MD recommends the sample pack for people who haven’t taken ED medication before. The sample pack offers more pills than a regular prescription at a lower cost. The total cost for this 30-tablet, one-month supply of 20 mg sildenafil is $60. This sample pack costs the same as the generic Cialis one-month supply of six tablets.

How to sign up for Rex MD

Like most online platforms for ED medication, the sign-up process includes a brief questionnaire. The questionnaire collects information about your medical history and symptoms. When we signed up, it took us about seven minutes to complete.

At the end of the questionnaire, you can choose between brand-name and generic Viagra or generic Cialis. If you are unsure which medication to choose, you can click on “Help me choose Viagra or Cialis.” An informational pop-up box lists a few things you can consider when choosing, including how long the medications last and when you should take them. We appreciate that it offers information to help you decide, but it isn’t as thorough as other platforms like Hims. Rex MD doesn’t provide any guidance on how to pick your dosage, but when you choose your medication, it tells you which dosage is most common.

Once your questionnaire is complete, a health care provider will review your answers and medication choice and approve or deny your request within 24 hours. They may modify your dosage based on your answers. If approved, your medication will be shipped for free within two to four business days.

You can reach Rex MD customer service through the following methods:

When we emailed customer service, we received a reply 26 hours later. Although one day isn’t too long to wait for an email response, other platforms offer instant chat on their websites, so you can receive a response much faster.

On Trustpilot, many reviewers reported having a positive, helpful experience with Rex MD’s customer service representatives on the phone. If you need to contact customer service, we recommend calling them for immediate assistance rather than waiting over one day for an email response.

Credit card payment options include Visa, MasterCard, American Express, PayPal, and Diners Club. You can also use a health saving account (HSA) and flexible spending account (FSA) card.

How we research and evaluate erectile dysfunction platforms

Based on our expert consultations, research, and testing, we determined the following factors to be essential for our readers when signing up for an erectile dysfunction medication platform:

We had all of our reviews medically reviewed by an expert in the field to ensure the information is appropriate for NCOA readers’ needs. Read more on our methodology here.

Bottom line

While Rex MD offers fast, discreet shipping and free messaging with your provider, we found it to be more expensive than other online platforms, and comparing the costs of the medications was confusing. However, we appreciated that they try to turn away users who shouldn’t be taking ED medication, such as people who take nitrate medications or have used certain recreational drugs within the last three months.

After researching 25 online platforms offering ED medication, Rex MD isn’t one of our top recommendations. We liked some of the platform’s features, but based on our research, you can find more affordable ED medications through other platforms like Hims.
