Older Veterans have substantial disease burden as compared to civilians. About half of Veterans are aged 65 and older. However, only one-third of Veterans aged 65 and older use the Veterans Health Administration (VHA) for health care, and many use both VA and community-based health and social services. Older Veterans may not be accessing community-based or VA benefits or services for which they are eligible and which may help promote positive physical and mental health in late life. Listen to this event to hear VHA experts discuss the importance of engaging Veterans through community-based services. Examples of successful partnerships to implement Chronic Disease Self-Management Education programs for Veterans, developed by state and local organizations in Utah and Maryland, will be highlighted.
