MAC, Inc. Living Well Center of Excellence began implementing the Living Well with Hypertension 2-1/2 hour Session Zero and measurement of blood pressure at Session Zero and at the end of chronic disease self-management education (CDSME) programs in 2015. Health care partners valued tracking of hypertension risk and documentation of reduced blood pressure for those with hypertension. Based on this success, the Center and partners developed the Stepping Up Your Nutrition Malnutrition Session Zero to assess malnutrition risk in community-dwelling older adults, as well as to measure hand grip strength (a validated measure for malnutrition) pre-/post- Stepping On or CDSME workshops.
