There are approximately 140,000 individuals living with Alzheimer’s disease in the Commonwealth of Virginia, and this number is projected to increase to 190,000 by 2025. At the Department for Aging and Rehabilitative Services (DARS), the agencies statewide initiative Live Well, Virginia!, which includes Chronic Disease Self-Management Education and A Matter of Balance, has been expanded to include persons with early-stage dementia (PWED) and their care partners. Listen to this presentation to learn about strategic partnerships, the referral process, and outreach efforts to engage PWED and their care partners including lessons learned, challenges, and successes.
