“Aging in place” has long been a goal of the aging network, with significant focus getting right services in the home:  home care providers, visiting nurses, and others. Less importance has been paid to the integration behavior change and self-management supports for home bound clients or clients uncomfortable with a group setting. This presentation highlights how one area agency on aging trained “1:1 program specialists” to provide multiple interventions in one visit: Enhanced Wellness, Healthy IDEAS, and falls assessments in the home for those who are unable to attend community programs or who express a preference for individual interventions.
