Chronic Care: Making the Case for Ongoing Care

This Robert Wood Johnson Foundation report examines the impact of chronic conditions on individuals and their caregivers, as well as on the U.S. health care system. The updated data is used to highlight the current problems encountered by individuals living with chronic conditions as they attempt to obtain a continuum of services in a health care financing and delivery system primarily oriented to the provision of acute, episodic care. It shows improvement in some areas but a growing prevalence and cost of chronic care.
