Access to transportation is critical to ensuring that older adults and adults with disabilities are able to actively manage their health. Mobility provides a way to see the physician, go to the grocery store, attend a social event, or participate in an evidence-based Chronic Disease Self-Management or falls prevention program. Join this webinar to learn more about resources available from national centers funded by the U.S. Department of Transportation, Federal Transit Administration including the National Center for Mobility Management and the National Aging and Disability Transportation Center. Learn how you can participate in mobility management networks to help individuals in your community access evidence-based health promotion programs and other health services. Examples of mobility management initiatives from the field will be shared.
