This roadmap is designed to help aging, public health, and disability professionals make stronger linkages with the health care sector to achieve better health through better care through evidence-based programming. It provides an interactive journey with various routes that you can choose to get to the destination of community-integrated health care.
This toolkit is designed to foster the expansion and sustainability of evidence-based health promotion programs by providing a centralized location for sharing resources. It contains a compilation of resources collected from Administration for Community Living, Administration on Aging (ACL/AoA) Chronic Disease Self-Management Education (CDSME) and falls prevention grantees, former grantees, and other organizations implementing evidence-based programs.
As you’re exploring, keep in mind that resources developed for one evidence-based program may provide valuable ideas for implementing or sustaining another program.
