Money Follows the Person

Money Follows the Person is a grant program that provides an enhanced federal Medicaid match to states that assist individuals in transitioning from nursing homes or other institutions back into the community.

The program was first established in 2005. However, the Affordable Care Act provides additional funding to expand it to more states and allow current states to transition more individuals to the community.
