Chronic Disease
Diabetes, arthritis, hypertension, lung disease. Chronic conditions make life unmanageable for millions of older adults — and force them to give up their independence too soon.
Today, 80% of older adults cope with at least one chronic disease — 50% more than one. Yet one-third leaves the doctor's office feeling confused about how to care for themselves.
One answer lies in self-care. Teaching people how to manage their symptoms to stay healthier longer. Our Center for Healthy Aging brings proven self-care programs to communities nationwide.
Kathy's Story
At age 55, Kathy was diagnosed with a rare, progressive lung disease. The doctor predicted she would die in six months. Instead of giving in, she got educated. Discover how a community program helped Kathy reclaim her life.
How We Help
- Programs that Work — Right in Your Community
We have the evidence. Self-care programs can give people with chronic conditions the tools they need to manage their symptoms and live independently. Find proven programs at our Center for Healthy Aging. - Getting Doctors on Board
Too often, doctors don't know how to meet the unique needs of older adults with chronic conditions. We're part of a fellowship program that gives doctors the chance to improve the way we deliver health care. Meet the fellows. - People Helping People
It's a model that works. We promote a program that trains people with chronic conditions to help their peers who are facing the same challenges. It's called the Chronic Disease Self-Management Program. Discover how it works.
