Partnerships

Partnerships are crucial at every stage of evidence-based programming.  Part of the planning process is identifying potential partners and forming the right partnerships to ensure the success and sustainability of your programming. • Partnering to Promote Healthy Aging: Creative Best Practice Community Partnerships
  A manual that provides insights and ideas for building state and local level partnerships between aging, health, and public health services that promote healthy aging.
• The Partnerships Analysis Tool: For Partners in Health Promotion
  The aim of this tool is to help organizations involved in health promotion projects to reflect on the partnerships they have established and monitor their effectiveness
• Community Partnerships for Older Adults
  This web site includes a Resource Center that showcases the issues involved with creating, maintaining and sustaining a community partnership. Resources are organized into categories including Strategic Planning, Inclusion & Diversity, Partnership Evolution, and Fiscal Strategies.
• Developing Effective Coalitions: An Eight Step Guide
  This guide helps partnerships launch and stabilize successfully. It supports advocates and practitioners in every aspect of the process-from determining the appropriateness of a coalition to selecting members, defining key elements, maintaining vitality, and conducting ongoing evaluations. • The Tension of Turf: Making it Work for the Coalition
  This guide offers practical support for skillfully managing the dynamic tension that commonly arises when people collaborate. Successful coalitions learn to anticipate and validate turf issues as part of solidifying partnerships and gathering their full strength.
• Promoting Older Adult Health: Aging Network Partnerships to Address Medication, Alcohol, and Mental Health Problems
  This publication is designed to promote new linkages between providers of aging services and relevant substance abuse and mental health services.
• Establishing Partnerships Between Managed Care and Aging Service Organizations

• RE-AIM for Program Planning: Overview and Applications
  This monograph introduces the RE-AIM framework. RE-AIM is an evolving framework designed to inform program decision-making by focusing on Reach, Effectiveness, Adoption, Implementation, and Maintenance. It includes examples of partnership in each of these elements.
