These PowerPoint slides feature activities that grantees from Massachusetts, Maryland, and Wisconsin have undertaken to develop a sustainable distribution and delivery system for CDSMP and other evidence-based programs. Strategies include the development of a health policy forum to raise program visibility and build a state coalition, leveraging Affordable Care Act opportunities to promote CDSMP and increase participant enrollment, and creating a non-profit institute to support and oversee evidence-based programs statewide.
