Risk Screening Medicare Members Revisited

This report addresses issues associated with the identification of high-risk members. As such, it builds upon an earlier report on the same topic issued in 1996. The report addresses three key areas. First, it discusses the strengths and weaknesses of existing self-report instruments used for newly enrolled members. Second, it discusses the potential for improving the health of their older members who suffer from specific geriatric syndromes. In the third section, provider referral, periodic risk monitoring using administrative data, and member or caregiver self-referral are discussed for risk identification of existing members.

This report is one of eight released by the HMO Workgroup on Care Management.
