In this webinar, Lisa Ferretti from New York’s Center for Excellence in Aging & Community Wellness, Melanie Mitros from Arizona’s Living Well Institute, and Brenda Schmidt from  Viridian Health Management will share how the Diabetes Prevention Program can provide opportunities for partnerships, potential for reimbursement by health care, and cross-referral to Stanford’s Diabetes Self-Management Program.
