Opening the Door to Partnerships with Health Care Organizations

Aging and disability network agencies have pursued a number of successful strategies to begin relationships with hospitals, physician groups, and other health care organizations. Evidence-based programs are often the key to unlocking what seem to be closed doors between the two worlds. Learn also about the IRS-required Community Health Needs Assessments for nonprofit hospitals and how to become involved to establish relationships. Finally, learn about national initiatives spreading innovative and successful approaches to building business opportunities to provide community-based services under contract with health care organizations.
