Opening the Door to Partnerships with Health Care Organizations

October 15, 2013

Aging and disability network agencies have pursued a number of successful strategies to begin relationships with hospitals, physician groups, and other health care organizations. Evidence-based programs are often the key to unlocking what seem to be closed doors between the two worlds. Learn also about the IRS-required Community Health Needs Assessments for nonprofit hospitals and how to become involved to establish relationships. Finally, learn about national initiatives spreading innovative and successful approaches to building business opportunities to provide community-based services under contract with health care organizations.

This webinar is part of a series given by the Center for Healthy Aging and partners as part of the U.S. Administration for Community Living, Administration on Aging's (through the FY 2013 Prevention and Public Health Fund) National Resource Center on Chronic Disease Self-Management Education Programs.
