In this presentation Gerry Mackenzie and Andrea Brandsness, NJ Department of Health and Senior Services, share the strategies that play a key role in sustaining evidence-based programs, including statewide and national partnerships and embedding programming into Title III-D and other government-funded programs to ensure funding/reimbursement for CDSMP.  Key strategies include: connecting with other evidence-based prevention programs, such as Matter of Balance; embedding training within university nursing curricula; providing programs in state prisons; and a statewide partnership with several state associations.

This webinar is part of a series given by the Center for Healthy Aging and partners as part of the Recovery Act Chronic Disease Self Management Program grant
