Medication Safety

Medication-related problems and errors are a growing and very common issue, especially with older adults. Serious medical issues can result from improper use of all types of medications.

In this webinar you can learn about medication safety among older adults and how the HomeMeds program can help. HomeMeds is an evidence-based, technology-enabled intervention that addresses medication safety among older adults by connecting homecare and other community-based services to health care providers. The program meets the Administration on Aging’s highest-level criteria for evidence-based programs and is complementary to other evidence-based programming.

Speakers:

  • Sandy Atkins, VP, Institute for Change, Partners in Care Foundation
  • Dennee Frey, PharmD, Project Consultant, Partners In Care Foundation

This webinar is part of a series given by the Center for Healthy Aging and partners as part of the Recovery Act Chronic Disease Self Management Program grant.
