Improving the Care of Older Adults with Common Geriatric Conditions

The goal of this report from the HMO Care Management Workgroup is to draw from a composite of evidence-based literature, best practices, and professional judgment to demonstrate how Managed Care Organizatoins can implement effective interventions that enhance functional status and quality of life for their older members. The report covers the following topics: physical inactivity, falls, medication-related complications, dementia, depression, undernutrition, and urinary incontinence. A summary of the report is also available.

This report is one of eight released by the HMO Workgroup on Care Management.
