Geriatric Case Management: Challenges and Potential Solutions in Managed Care Organizations

This report seeks to generate a broader understanding of managing care for high-risk populations by describing the challenges and opportunities in developing and implementing case management programs. The report is intended to be particularly useful for case management programs that serve “multi-problem” and frail older adults. The report does not pertain to programs that address a single overarching problem (i.e., disease management programs) because their application to the management of older adults with complex needs is likely to be limited.

This report is one of eight released by the HMO Workgroup on Care Management.
