Building Bridges: A Business Approach to Partnerships with Accountable Care Organizations and Health Systems

This webinar will detail and examine the strategy used by the Detroit Area Agency on Aging to successfully build a bridge between the agency and the Detroit Medical Center and their Pioneer ACO, allowing the agency to provide care coordination and care transition services to their patients and beneficiaries. Specific discussions will explore the importance of relationship building, understanding issues related to quality metrics, and identifying “value-added” delivery models. The skills learned in this session will also apply to other emerging partnerships such as managed care organizations, including those who offer managed care for dual eligible population.
