Evidence-based Disease and Disability Prevention Program

Older Americans are disproportionately affected by chronic diseases and conditions, such as arthritis, diabetes and heart disease, as well as by disabilities that result from injuries such as falls. Funding support to states under the Evidence-Based Disease and Disability Prevention programs (EBDDP) was provided by AoA to empower older adults to take control of their health. Under the EBDDP grants, 24 grantees across the country were funded to support dissemination of evidence-based programs in their communities. This report is based on data derived from the states‘ Final Reports, interviews with stakeholders, and additional NCOA resource center materials.

American Recovery and Reinvestment Act Communities Putting Prevention to Work: Chronic Disease Self-Management Program

Self-management programs provide older adults with education and tools they need to help them cope with chronic diseases such as diabetes, heart disease, lung disease, or arthritis. In March 2010, AoA provided funding support to 45 states, the District of Columbia, and Puerto Rico under the American Recovery and Reinvestment Act (ARRA) Communities Putting Prevention to Work: Chronic Disease Self-Management Program (CDSMP). This program enables older Americans with chronic diseases to learn how to manage their conditions. This report is based on data derived from the states‘ Final Reports, the NCOA CDSME National Database, and additional NCOA resource center materials. The report covers the 39 states with grant periods that ended by September 30, 2012.
