CVS Award Winners Share Best Practices: Part II

Three of the six winners of the 2008 CVS Prescription for Better Health and Wellness Award shared their best practices in a recent Webinar.

Our presenters include the following:

Cil Nicholson of  Mecosta County Senior Center (Mecosta, Mich.) – Personal Action Toward Health (P.A.T.H.) is an evidence-based, eight-week class created by the Stanford University Patient Education Center and made available to Mecosta County Senior Center through Area Agency on Aging, West Michigan.  The class is both educational and experiential.  Participants learn how to set goals, design action plans, and problem solve.  They also learn skills, such as muscle relaxation and breathing techniques.  As a result of the success of the program, The P.A.T.H. class is now state-wide in Michigan.

Leona Lavin of Grantsville Senior Center (Grantsville, Md.) – “Standing Strong: Evidence Based Falls Prevention Program” is a six-week health education program aimed at falls prevention for
adults aged 60 and over.  The program is comprised of five components: orientation, balance screenings, home modification for safety and falls prevention, medication management, and balance exercises. 

Bonnie Wilker of Two Rivers Senior Center (Two Rivers, Wis.) – The goal of  “Breakfast for your Brain” is to encourage seniors to realize the benefits of consistent mental exercise combined with a commitment to physical activity and good nutrition.  The 20 initial sessions have been designed for use in a stand-alone format, each session taking approximately one hour to complete.  Assessment tools provide outcome measurements.  The session focuses on areas such as nutrition, how your brain works, humor, and memory building exercises.
