CVS Award Winners Share Best Practices
April 24, 2008

Three of the six winners of the 2008 CVS Prescription for Better Health and Wellness Award shared their best practices in a recent Webinar.

Our presenters include the following:

Madison Senior Center (Madison, N.J.)

The award-winning “Stay Healthy” program, which is designed and conducted by volunteer, certified gerontological nurse, Dr. Sandra Fielo, helps elders adjust to life with chronic disease and the inevitable changes associated with aging. Dr. Fielo collaborates with the center’s coordinator and Madison Health Department to provide free health-related consultations, encourage participation in community activities, and promote a proactive stance on staying healthy. 
 
Overview:

  • Components of the Stay Healthy program are based on a health promotion strategy 
  • Benefits to clients include increased functioning, socialization, ability to make informed medical choices and participation in community activities 
  • Steps in instituting a Stay Healthy program

TMC Senior Resource Center (Tucson, Ariz.)

“Healthy Living Connections” is a program that offers classes and lectures on health topics, health screenings, access to a health information center, and trained volunteer health coaches so seniors can learn ways to prevent and manage any health condition and improve their quality of life. Trained volunteer health coaches provide one-on-one support to help older adults set health goals and stay motivated to improve their lifestyles.

Cabarrus County Senior Center (Concord, N.C.)

The Wellness Report Card is a measurement tool used to evaluate fitness programs as part of  “Healthy Lives, Healthy Futures” (HLHF). HLHF is the name of the Cabarrus County Senior Center’s comprehensive wellness program that focuses on behavior change in the following areas: smoking cessation, nutrition and fitness. The Wellness Report Card aims to measure and record changes in blood pressure, BMI, inches lost, cardiovascular endurance, lower body strength, and flexibility. This tool is also a motivation strategy in that it is given to participants in the form of a booklet to keep as a record of their progress.
