Overview:

This session discusses the Chronic Disease Self-Management Program (CDSMP), developed by Kate Lorig and associates at Stanford University.  Using the Stanford model, Partners on the P.A.T.H. CDSMP in Western Michigan is one of 14 evidence-based prevention projects nationwide supported by a three-year grant from the Administration on Aging.

Speaker:

Bonnie Hafner, BSN, RN, is the speaker. She is a registered nurse at the Area Agency on Aging of Western Michigan, where she has served as the Quality Assurance Nurse for Care Management and MI Choice Waiver programs for the past seven years. Ms. Hafner has more than 28 years of experience working with older adults and caregivers in a variety of settings, including geriatric and mental health assessment, public health, long-term care, education, and consultation. 

Ms. Hafner also is the Project Coordinator for Partners on the P.A.T.H. (Personal Action Toward Health), an Administration on Aging-funded evidence-based prevention project that brought the Stanford Chronic Disease Self-Management Program (CDSMP) to older adults in a west Michigan county. She is currently on a workgroup charged with the statewide dissemination of P.A.T.H.
